interRAI Home Care for Children and Youth (HC-CY) Assessment Form and User’s Manual, interRAI Standard Edition, 10.0
The interRAI HC-CY Assessment System is designed for use in home care for children and youth between the ages of 4 and 21 years with health care chal¬lenges or who are receiving services from the child and youth service sector. The spectrum of young people with such challenges is broad — for example, children born with congenital abnormalities, young people with developmental disabilities or special health care needs, and youth with neurological conditions who live in the community but often depend on technology such as ventilators, feeding tubes, and other medical devices. The instrument is designed for use by home care providers and to include services provided in the home and at school (where applicable). The interRAI HC-CY Assessment System may be used in other settings such as in-patient or outpatient rehabilitation centres and special school settings. IMPORTANT NOTE ON THE INTERRAI HC-CY VERSUS INTERRAI PEDS-HC: The interRAI HC-CY Assessment System used the framework of the interRAI PEDS-HC Assessment System but incorporated the use of some Collaborative Action Plans (CAPs) and scales developed for the interRAI Child and Youth Mental Health (ChYMH) and interRAI Child and Youth Mental Health and Developmental Disability (ChYMH-DD) Assessment Systems (which were published more recently than the interRAI PEDS-HC). This book contains introductory material, an item-by-item guide for using the interRAI HC-CY assessment form, and the assessment form itself.
