Abstract: 

To describe how case managers can adapt their assessment practices to intentionally engage people living with aphasia in both the assessment and care planning process. Home and community care. Case managers can support meaningful engagement for people living with aphasia by using evidence-based strategies and aphasia-friendly communication. A skilled, encouraging assessor amplifies the voice of the person with aphasia through a person-centered assessment and care planning process incorporating client input, caregiver insights, observational cues, and clinical documentation.
